How Do I Apply
FOR NEW STUDENTS: All new families who would like to attend Holy Savior Academy must fill out a New Student Admission Application form and return it with the required documents listed below and fees.
Fees
Registration Fee (Non-Refundable and made out to Holy Savior Academy)
$100.00 per Family Before March 1, 2020
$250.00 per Family After March 1, 2020
Academic Fee (Kindergarten- Grade 8) (Non-Refundable after Aug. 1st)
$ 350.00 per Student Before June 1, 2020
$ 400.00 per Student After June 1, 2020 (Paid directly to HSA
Or billed thru FACTS in August payment)
Facilities Fee (Kindergarten- Grade 8)
$500.00 per Family (Billed thru FACTS if not received by August 1)

- New Student Admission Application completed, signed and dated
- PreK Contract, signed and dated
- 2021-22 Kindergarten – Grade 8 Contract, signed and dated
- Birth Certificate
- Baptismal Certificate (Catholic Only)
- Communion Certificate (Catholic Only)
- FACTS Automatic Tuition Payment Agreement Enrollment online
- Full Tuition Payment Check to Holy Savior Academy by August 1st
- NJ Department of Education Physical Evaluation Form, Parts A and B
- Immunization Record Form (Due at Registration)
- B6T Transportation Form (Kindergarten- Grade 8)
- Authorization for Student Record Release Form (Kindergarten- Grade 8)
- Request for Loan of Textbook Form (Kindergarten- Grade 8)
- Proof of custody from legal divorce, separation, single parent/guardianship
